Health Insurance
General Information
Student health insurance is available to USU students if they are enrolled in 6 or more undergraduate credit hours and also to those enrolled in 1 or more graduate credits. Undergraduate students can find out more in the Unsubsidized Health Insurance and Links sections further down the page. Rules for graduate student insurance are outlined below.
Graduate Student Requirements
All other graduate students are required to have a health insurance plan if:
- you are working 0.5 FTE (20 hours/week) or
- you have a scholarship/fellowship of $10,000 or more
Students may choose to enroll in a health insurance plan through USU or through an outside company. If you choose to enroll outside of USU, you must provide evidence that you are covered by an equivalent policy. Details of health insurance through USU are described below.
Subsidized Health Insurance
Subsidized health insurance is offered only to graduate students who meet the following qualifications:
- full-time registration for graduate study and have at least one of the following:
- 0.5 FTE (20 hours/week)
- scholarships/fellowships of $10,000 or more.
Cost of Subsidized Insurance
- Graduate student:
- Fall/Spring attendance: $237 annual
- Family members:
- Spouse: $2,911 annual
- All Children: $2,208 annual
- Regardless of your attendance pattern, you will be covered for 12 months
Deadlines
- September 4: Students are assessed $237 on their student account
- September 11: Premiums are due from students
Unsubsidized Health Insurance
If you do not meet the qualifications for subsidized health insurance, you may still enroll through USU. A packet of information and enrollment forms may be picked up at the Student Health & Wellness Center or the Graduate Office. You can also register online at www.firststudent.com.
Insurance is only mandatory for those that qualify for the subsidized program and international students
Cost of Unsubsidized Insurance
- Student: $1,177 annual
- Spouse: $2,911 annual
- All Children: $2,208 annual
