Using the USU Reporting Hotline

The purpose of the USU Reporting Hotline is to help protect the university community, including its students, employees, visitors and assets.

The USU Reporting Hotline allows anonymous reporting, helps promote confidentiality, and provides safeguards against retaliation. Good-faith reports of regulatory or policy non-compliance will be reviewed and, if appropriate, investigated and resolved. 

The USU Reporting Hotline provides a way to:

  • Report suspected noncompliance with USU policy or other governing laws or regulations, particularly when you are uncomfortable reporting these matters through other reporting lines
  • Address compliance issues that have not been resolved through other direct internal resolution mechanisms
  • Ask general questions about compliance matters

What NOT to Report Using the USU Reporting Hotline

The USU Reporting Hotline is designed to address issues of noncompliance. General complaints and grievances should not be reported through the USU Reporting Hotline, but instead, reported in the following way:

  • General employee complaints and grievances (e.g., performance evaluations, work assignments, wages, or merit raises) should be directed to the employee's supervisor and/or USU Human Resources.
  • Student academic grievances should be directed to the student's instructor, the dean of their academic discipline, and/or file an academic grievance.
  • Academic freedom or faculty tenure and promotion grievances should be filed directly with the Academic Freedom and Tenure Committee.

If such issues are reported through the USU Reporting Hotline, they will be assessed by the hotline administration team and referred to the appropriate unit for further action.

How To Make a Report

You can submit a report using either of the following methods:

  1. Online: Click the "Make a Report" button on this webpage.
  2. Phone: Call toll-free within the United States, Guam, Puerto Rico, and Canada at (844) 916-2760.

Important Instructions When Reporting through the USU Hotline

  • Please provide clear and complete information so that USU can properly investigate.
  • Check back regularly in case follow-up questions are posted in the system.
  • EthicsPoint is USU's third-party hotline provider, and they do not track anonymous reporters in any way.
  • To confirm that your concern was received or to ask follow-up questions, we recommend that anonymous reporters keep the report information and follow up regularly after making a report.
  • After you complete your report, you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place.
  • After 5-6 business days, use your report key and password to check your report for feedback or questions.

Privacy Notice

Utah State University (USU) is committed to protecting your privacy and ensuring the confidentiality of any information you provide through our Reporting Hotline, EthicsPoint, managed by NAVEX Global. This applies to reports made online or via phone.

When you make a report, we may collect personal data including but not limited to:

  • Contact information (not required)
  • Details of the incident or concern
  • Any supporting documentation

Confidentiality

Your information will be used solely to address and investigate the reported concerns. Information provided may be shared with relevant personnel within USU, such as compliance officers or legal advisors, strictly on a need-to-know basis. We may also share data with external authorities if required by law.

You have the option to submit reports anonymously. When a report is made anonymously the university will strive to protect your identity and the confidentiality of the information provided to the extent reasonable and as permitted by applicable law and policy. If a report is made anonymously, please know that the relevant unit will review the report and, if appropriate, investigate based on the information contained in the report.

We implement appropriate technical and organizational measures to protect your personal data against unauthorized access, alteration, or disclosure.

You have the right to access, correct, or delete your personal data, as well as to object to or restrict its processing. For any privacy-related inquiries or to exercise your rights, please contact the USU Chief Privacy Officer.
