Uploading Forms
If you have ongoing verification or appeal processes, please upload documentation to your StudentForms account.
- Forms must be in a .pdf format. Other formats can't be opened or reviewed.
- Please keep your student account updated with your current email, phone, and address. Make sure to check your USU email periodically.
- Notarized forms cannot be uploaded to Box, please mail them instead and allow 1-2 business days for receipt, and 1-2 weeks for processing.
