Graduate Student Forms
- Fill out the student annual review form and submit it to your major professor via this ServiceNow link. Your major professor will also fill out a review form and submit it via ServiceNow. This review will be saved to your student files.
- Supervisory Committee Approval Form (SCAF): MS/CS Plan A, MS/CS Plan B, MS/DS Plan A, and PhD/CS
- Program of Study (POS): MS/CS Plan A, MS/CS Plan B, MS/DS Plan A, MCS, and PhD/CS
- Master's Thesis/Project Proposal Approval (TPA): MS/CS Plan A, MS/CS Plan B, MS/DS Plan A
- Application for Candidacy (ACDD): PhD/CS
- Appointment for Examination (AFE): MS/CS Plan A, MS/CS Plan B, MS/DS Plan A, and PhD/CS

- Plan B Report/Creative Project Submission: MS/CS Plan B
- IRB-approved Protocol Closeout: If your thesis/report/dissertation requires IRB approval.
- Embargo Request: If you/your advisor would like to restrict electronic release of your thesis/dissertation.
- Graduation Check Request: MS/CS Plan A, MS/CS Plan B, MS/DS Plan A, MCS, and PhD/CS
