USU Facilities Customer Service & Key Office
Customer Service is the communication, dispatch, and information center for the USU Facilities department. We assist with work order requests, key and access card requests, and information about Facilities-related projects. Issues dealing with building infrastructure, heating or cooling, roof leaks, custodial, moving requests, etc. may be reported and coordinated by calling or submitting the appropriate request form.

For building access, talk with your department Access Coordinator and/or a department head to begin the request process.
The USU Key Office is not permitted to issue keys or access cards without a request from the department.
The Key Office follows all of the guidelines outlined in University Building Access Control Policy 520.
Re-key requests can be made in the event of:
- Loss of safety to a space (e.g. lost keys)
- Departmental move
Requests are made by completing the door re-key request form, including required signatures, and emailing it to facilitiescustserv@usu.edu. Cost is paid by the requesting department.
Work orders are for building and space repairs. This includes:
- Restoring malfunctioning items
- Minor alterations such as white board installation, additional electrical outlets needed, doorstop installation, etc.
- Moving services
Use a Facility Modification Request for larger modifications, equipment upgrades, facility improvements, or a request that needs cross-team coordination.

Facility Modification Requests are for large space-modifications and projects involving cross-team coordination. This includes, but is not limited to:
- Change in space use (e.g. transforming a large space into multiple smaller spaces)
- Wall additions or removals
- Window additions or removals
- Doors additions or removals
- Large equipment installation
- Renovations and upgrades
Requests will be reviewed by the appropriate Facilities projects teams. If approved, design work and proper estimates will be compiled to ensure compliance with building codes and university standards for construction.
Modifications, upgrades, or facility improvements to buildings at USU are not permitted without proper approvals from Facilities.
Projects are coordinated with the Fire Marshall, Space Committee, Information Technology, etc., as needed.

The university has a standardized sign system for all permanent signs used on the interior of buildings. This ensures uniformity, way-finding, and compliance with the Americans with Disabilities Act (ADA).
New sign locations and content are subject to USU Sign Committee review.
To request a new sign, complete an Interior Signage Request Form, including all required signatures, and email to facilitiescustserv@usu.edu.
USU Facilities maintains and installs artwork that may be hung from banner poles throughout the Logan Campus.
