Vaccinations and Immunizations are free for USU-benefited Employees and their Eligible Dependents. 

Covid Vaccines Also Available
Pfizer COVID-19 vaccines will also be available for individuals 12 years and older. Please download, print, complete, and bring the additional COVID-19 Patient Information Sheet for each person receiving the vaccine. Booster doses should be available for those that meet CDC recommendations. Bring your COVID-19 vaccine card with you to the clinic for verification. Registration is required to participate and masks are encouraged for all ages 2 years and older. If you are sick or not feeling well, please stay home and do not attend the Immunization Clinic. For questions about registration, please call 435-797-0122.

Register

Register for an available time slot for the clinic and please include which vaccines you want to receive.

Complete the Information Sheet

For each registrant, download, print, complete and bring the Patient Information Sheet with you to your Clinic appointment.

Vaccine Information

The following vaccines will be available:

  • COVID-19 (1st, 2nd, and 3rd doses available)
  • Hepatitis A and B (can be given together)
  • HPV, or Human Papillomavirus (approved through age 45)
  • Influenza
  • Measles, Mumps, and Rubella (MMR) Booster (suggested for those born between 1957 and 1989)
  • Meningitis (for those who travel outside of the US)
  • Meningitis B (targeted for college students between ages 18-25)
  • Pneumonia (especially for ages 65+)
  • Shingles (for ages 50+)
  • TDAP (tetanus, diphtheria, pertussis)

Can't make it to a clinic?

If you are unable to make it to one of the annual on campus immunization clinics you may receive free vacinations for you and your dependents at any pharmacy within the Regence BlueCross BlueShield of Utah network. When visiting a pharmacy be sure to show your Regence insurance card. View a list of Regence Preferred Network pharmacies.

Important Reminders

  • You must register for yourself and ALL dependents receiving immunizations at the Clinic.
  • Bring your Blue Cross Blue Shield (BCBS) insurance card to the Immunization Clinic.
  • Masks are welcome for all ages 2 years and older.
