Academic Record Adjustment and Reversal of Tuition and Fees Petition

The record adjustment provides the opportunity for you to petition for changes to your academic transcript (credit adjustment, course addition, withdrawal with "W" notation, etc.). A “W” (withdrawal notification) is a registration status and is not calculated into your GPA.

The request for reversal of tuition and fees allows you to petition for a reversal of tuition and fees or for a forgiveness of debt owed outside of normal refund deadlines. If a reversal is being requested for a graded or incomplete course, the student must also request an academic record adjustment to request that the course grade be changed to withdrawal status 'W'. If in this situation the academic record adjustment is denied, the reversal will also be denied.

A maximum of two semesters may be adjusted per degree. No adjustments will be made after a degree has been awarded. Requests must be submitted within two years of the semester for which there is a desired adjustment. 

Adjusting your academic record may result in changes to tuition, fees, financial aid, and scholarships, potentially leading to an outstanding balance on your account. Please carefully consider these potential consequences before submitting a petition.

Petitions are only reviewed if a student does the following:

  1. Demonstrates an extenuating circumstance beyond the student’s control.
  2. Demonstrates the extenuating circumstance occurred during the semester for which the student is petitioning.
  3. Includes all supporting documentation in the petition.

Extenuating Circumstances

An extenuating circumstance is a non-academic emergency outside of your control that occurred during the semester for which you are petitioning and affected your ability to attend or fully participate in your classes. As a USU student, you are responsible to be aware of and comply with all USU registration dates and deadlines.

Petition Process

  • A personal statement describing your situation, how it impacted your ability to attend or participate in courses, and specific dates for when the events occurred.
  • Supporting documentation for your situation
  • After your petition is submitted, a $20 non-refundable petition processing fee will be posted to your account. Fee must be paid for petition to be reviewed. (Only applies to those petitioning for a record adjustment).
  • Check your official USU email account. All communication regarding your petition will be emailed to you.
  • If you are pursuing an Academic Record Adjustment (ARA) due to the impact of sexual misconduct or discrimination, the Supportive Measures Specialists in the Office of Equity can help you navigate this process. You can schedule a meeting with a Supportive Measures Specialist or learn more.

Supporting Documentation

  • You will need to download the Medical Verification Form and have your licensed care provider fill it out. They can also write a letter (on letter head) however the Medical Verification Form is preferred as it contains the specific information the committee requires.
  • Your licensed care provider must indicate that your condition or illness prevented you from attending classes for a time period of two weeks or longer (or one week for a 7-week session course).
  • How to Submit the Form: Your licensed care provider can submit the verification form to our office via fax at 435-797-1110 (Attn: ARA Committee) or mail to 1600 Old Main Hill Logan, UT 84322-1600 Attn: ARA Committee. They can also return the form to you and you can attach it via the ServiceNow forms.

  • You will need to download the Medical Verification Form for a Family Member and have your licensed care provider fill it out. They can also write a letter (on letter head) however the Medical Verification Form for a Family Member is preferred as it contains the specific information the committee requires.
  • Your licensed care provider must indicate that your condition or illness prevented you from attending classes for a time period of two weeks or longer (or one week for a 7-week session course).
  • A family member (including step-relatives) for this policy is defined as a student’s spouse, son, daughter, son-in-law, daughter-in-law, brother, sisters, brother-in-law, sister-in-law, grandparents, grandparents-in-law, grandchildren, or domestic partner.
  • How to Submit the form: Your licensed care provider can submit the verification form to our office via fax at 435-797-1110 (Attn: ARA Committee) or mail to 1600 Old Main Hill Logan, UT 84322-1600 Attn: ARA Committee. They can also return the form to you and you can attach it via the ServiceNow forms.

  • You will need to download the Employer Verification Form and have your current employer fill it out. They can also write a letter (on letter head) however the Employer Verification Form is preferred as it contains the specific information the committee requires.
  • Your employer must verify that there was a significant change in work hours required by them and that directly conflicted with your course schedule and ability to attend or fully participate in class. New employment is generally considered the student’s choice and therefore would not be considered an extenuating circumstance.
  • How to Submit the form: Your employer can submit the verification form to our office via fax at 435-797-1110 (Attn: ARA Committee) or mail to 1600 Old Main Hill Logan, UT 84322-1600 Attn: ARA Committee. They can also return the form to you and you can attach it via the ServiceNow forms.

  • You will need to turn in a death certificate, obituary, or funeral program to document the event. A death certificate can be submitted without any further documentation. An obituary or funeral program require that your name be listed in the documents as proof that you are related to the deceased family member.
  • A family member (including step-relatives) for this policy is defined as a student’s spouse, son, daughter, son-in-law, daughter-in-law, brother, sisters, brother-in-law, sister-in-law, parents, grandparents, grandparents-in-law, grandchildren, or domestic partner.
  • How to Submit: You will attach the document via the ServiceNow forms.

  • You will need to turn in your proof of jury duty or subpoena.
  • How to Submit: You will attach the document via the ServiceNow forms.

  • You will need to turn in documentation with dates, signatures, etc. that indicate the extenuating circumstance and why it inhibited you from attending or fully participating in your classes for a time period of two weeks or longer during the semester (or one week for a 7-week session course).

Submit a Petition

REQUIRED: Please submit a complete petition with all supporting documentation. Incomplete requests will not be accepted. After all documentation has been submitted, please allow 2-3 weeks for your petition to be processed. 


Status Updates on a Submitted Petition

You will receive an email notifying you of the status of your petition and of any additional steps you need to take, such as paying the petition fee, adding additional documentation, and more. If you have successfully submitted the form, meeting all of the requirements, the committee will review it within two to three weeks. Once the review is complete, you will be notified of the results via email within a week.
