Tier 3 Facility Reservation Form

For use by community entities, or USU departments facilitating non-mission critical programming

Tier 3 users groups are classified as Community Entities, or USU-Departments facilitating non-mission critical programming. All Tier 3 event bookings require a signed facility use agreement or liability release waivers before utilizing Campus Recreation facilities; these documents will be provided with your confirmation after your scheduling request has been received and processed. Please direct additional questions to campusrec@usu.edu.

Tier 3 requests must be submitted 3 weeks prior to the requested event date. Requests submitted outside of this window may be deemed unserviceable. Submitting a request does not guarantee a reservation. Requests are not approved until a confirmation email from the scheduling team has been issued.

By submitting this form, the User and User Party agree to defend, indemnify, and hold harmless the University, its agents, or employees from any and all injury, damage, or loss to persons or property that results from or is in any way connected with the use of University property by the User, User Party, guests, invitees, volunteers, etc., except where such injury, damage or loss is caused by the sole negligence of the University.
